So, I found out I have a condition called Telogen Effluvium and I need a bit of a break :( my life prior wasn`t the best and cause some trauma to my system. Acute telogen effluvium has an abrupt onset. The active shedding lasts 1 to 4 months and can lead to thinning of the hair over the entire scalp. The bitemporal areas may be most obviously thinned. Once the daily shedding returns to normal, it may take several months for the scalp hair to regain its normal density. The patient’s hair goes back to normal within 1 year of onset of TE in 95% of people. A specific trigger can be found in approximately 75% of patients with acute TE. Chronic TE is defined as increased shedding lasting at least 6 months, but usually greater than one year. Chronic TE can last for many years with fluctuating severity. Its onset may be abrupt or gradual. A definite trigger is often not identified in patients with chronic TE. Chronic TE usually affects middle-aged women, many of whom give a history of extremely thick hair prior to the onset of their problem. Women with chronic TE may have normal-appearing hair (albeit thinner than the patient’s normal hair) or may show shorter frontal hairs or bitemporal thinning. Women with chronic TE often state that they have lost half or one third of their hair. A woman with chronic TE may complain that she is unable to grow her hair as long as she could in the past. Unlike in FPHL, the occipital and crown part widths are approximately the same in patients with both acute and chronic TE.

I recently had a Trichology Consultation, and if you didn't know, Trichology is the branch of medical and cosmetic study focused on hair and scalp. I only recently learned that this field of study existed, and that I have a really great Trichologist nearby! In our assessment a few interesting facts came to light, all suggesting that my hair loss may be due to other underlying factors and not alopecia. It seems that genetically I am less equipped to deal with any form of hair loss in general. But it's not 100% clear that Androgenetic Alopecia is the cause. I am currently dealing with Telogen Effluvium due to stopping a number of hair loss treatments, so it will take a few months, maybe even a year before we can really narrow things down. Female pattern hair loss: This is a slowly progressive hair loss seen in women. It can affect women at any age. This is second part of the videos on hair fall in women. We discuss causes of hair fall and the condition telogen effluvium in detail. It occurs after 18 years of age mostly. In severe cases, it may be noticeable as early as 12 years. However, most of the patients who approach us for treatment are in the age group of 25-40 years. There is a second peak incidence at menopause, between 50 and 60 years of age What is the cause? The reason for this hair loss is thought to be due to multiple factors. It may be influenced by genetics, hormonal imbalance or even environmental factors. The age of onset, how it will progress, the severity that is whether it will be restricted to one area or involve the entire scalp and the pattern of hair loss, are all determined by genetics. multiple marriages, lack of photo protection, higher income and little physical activity. However, more studies are required to prove an association with these factors. Hormones have also been implicated in the causation. Though it has not been unequivocally proven. Especially androgens. It is thought that testosterones and DHT play an important role in hair loss. Also, the lack of estrogen with age is responsible. The levels of estrogen drop after menopause which is why we see this condition more after menopause. Women with FPHL usually first notice a gradual thinning of their hair, mostly on the top of their heads, and their scalp becomes more visible. Over time, the hair on the sides may also become thinner. The patient may notice that her “ponytail” is much smaller or that her longer hair looks “skimpy” at the ends. Males can suffer from hair loss and early as their 20's. In fact by the age of 50, 50% of males will experience hair loss. Normal hair growth takes place in three phases in the follicle: anagen, catagen and telogen phases. In the anagen phase, the cells of the hair matrix responsible for growth actively multiply and are pushed upwards to form the hair shaft. After two to six years the follicles transitions into the catagen phase. In this phase hair growth stops. The base of the hair shaft keratinizes and moved upwards away from the papillary blood supply. When the hormone (typically testosterone) binds to the androgen receptor. it triggers 5-alpha reductase to convert that testosterone into something called DHT. Its the DHT which plays the most active role in damaging hair production. This is why "inhibiting DHT" is such a big catch-phrase in hair loss circles. That, and "anti-androgens". Things which inhibit DHT, or work against this androgen process, are considered legitimate hair loss treatments. Treatments for Women with Androgenetic Alopecia : 1. A very common question asked by most of my patients is whether my hair loss can be reversed. Hair loss is a very common term which is used by common man and they usually say that they are loosing hair. But if you look at it medically there are 40 different types of hair loss and whether it can be reversed or not, depends upon the type of hair loss the person is shaving which we can diagnose with the help of certain equipments like videomicroscope or a hair analyser, equipments like that. There are biopsy, blood tests and all those things with which we can identify what type of hair loss it is and we can tell the patient whether you can reverse it back or not. Medically speaking among all the 40 types, we do not get all the 40 types of cases that come to us. The most common type of hair loss that comes to us androgenic alopecia, alopecia areata and telogen effluvium. They are the 3 most common types of hair loss that we come across. Now if you look at androgenic alopecia is a condition where the hormone dihydroxytestosterone which is there in both men and women comes and attaches to the hair roots on the crown area of their scalp because the scalp area becomes sensitive to this particular hormone. When the hormone attaches to the root if the hair the nutrition which is supposed to reach the hair reduces and hence the hair starts thinning. Everytime a thicker hair falls, a thinner hair starts growing, a thin hair falls a even thinner hair grows and eventually it dies. This process of thinning of the hair strand usually happens after 50 years or after 60 years but nowadays we see a 20 year or 30 year old people coming up with this common type of hair loss where the hair is thinning. This is an ageing process of the hair strand.and aeging is supposed to happen in a later on stage in life but for some people due to various lifestyle and reasons and hormonal problem and so many things like that we get this condition happening earlier. Now here in medical science, two things is irreversible, one is if I become old, I cannot become young. Once there is death, the dead tissues cannot be restored and once a person becomes old, he cannot become young. The same rule applies to the hair. Once the hair become sold, it cannot become young, once the hair dies, ten you cannot bring it back to life, there’s no way that you can do it. So in androgenic alopecia, which is a process of ageing, after it ages, it dies. So once it dies then you cannot bring it back to life. Once it ages, the thick hair what you have on the scalp can become as thin as what you have on the body, as small as the hair you have on my hand now here. Of these types of very small types of hair are also seen on the scalp as the scalp hair becomes thinner and thinner. So once it becomes very old like that you cannot make it thicker, there’s no way. Once it dies you cannot bring it back to life again. So in this condition, it is irreversible you have to arrest that particular process with treatment and the hair which has just started ageing, we can improve it, but the hair which has become old, we cannot. Like ion the other condition, telogen effluvium, it’s a common condition seen after pregnancy, after delivery, when women start loosing a lot of hair and maybe after a major illness like typhoid, malaria, jaundice, after antibiotics people loose lot of hair like in bunches. Like if I touch the patient’s hair, I get 10 – 15 hair in my hand. So that is telogen effluvium. In this particular type of condition, you get , you lose hair from all over your scalp, not specifically form the crown alone, its from the entire scalp and it is this type of hair loss is there, you can restore it back. Actually the patient starts improving. Many patients restore or improve it even without any treatment. That is called acute telogen effluvium. When it is chronic, when it goes beyond a particular stage like more than 6 months the person is losing hair like this, then its called chronic telogen effluvium and in this condition we have to interfere and we have to treat it with medication. Otherwise the person will go bald. The other type of alopecia is alopecia areata. Alopecia areata is a patchy hair loss which comes here and there in patches. In this type there are two types of hair loss, scarred and non-scarred. In the scarred one there is a scare which forrns in the area of the patch and the scar once it forms, it is like any other wound scar, their hair roots are destroyed and hair cannot grow back whereas in the non-scarred variety you can grow back he hair if you treat it and if you don’t treat it also in some patients it grows back. But in cases these patches starts increasing the entire hair of the scalp is lost, that is called alopecia totalis, the total hair is lost and some people, it even affects the eyebrows, it is an autoimmune condition.

What is Telogen Effluvium or TE? So, what is Telogen Effluvium exactly? Aside from being difficult to pronounce, it is also often mistaken for some sort of scientific terminology that pertains to something or other. Many people have never even heard of Telogen Effluvium, let alone know what it means. Therefore, to better understand TE, let's take a closer look at your hair follicle's growth cycle, as this is the aspect of your biology that is greatly affected by Telogen Effluvium. Firstly, it's important to know that the hair growth cycle has 3 phases: Anagen- This is the GROWTH phase. During this phase your hair will look healthy and full, as well as nourished, given that it is growing at a rate of around half an inch per month. Under normal circumstances, this phase lasts anywhere between 2 to 8 years and roughly 85% to 90% of your scalps 100,000 hair follicles will be in the Anagen phase at any one time. Catagen- This is the TRANSITIONAL phase. This phase only lasts for around 2 to 4 weeks under normal circumstances, though it is an extremely important step in new hair growth. During this time your hair will actually shrink in size and it will disconnect from the blood supply. At this point a new hair follicle will begin to grown from under your scalp. Approximately 5% of hair follicles are in the Catagen phase at any one time. Telogen- This is the RESTING phase. Now new hair growth happens at this point, as the follicles are simply waiting to emerge from the scalp. During this phase your hair follicle is at its weakest and it can easily be pulled out of your scalp. This phase will typically last for about 3 months and will end when your existing hair follicle falls out and makes way for a new one. Now that we've gone over the various hair growth phases, let's discuss how Telogen Effluvium effects its sufferers rate of new hair growth and can lead to shedding. Telogen Effluvium indicates that there is an outflow of hair that is in the Telogen phase. Think of it as your locks getting so tired from the stresses that they suddenly have to deal that they simply choose to die and fall out. A number of stresses can bring this about, such as a severe shock to the body or extreme trauma (a car wreck, break up, death of a loved one, extreme dieting, and even certain medications). Essentially, any type of shock to the body can bring on an onset of Telogen Effluvium. The main problem with Telogen Effluvium is that it is still a poorly defined condition. As mentioned before, very little research has been done on the subject, hence all of the confusion that surrounds it. For example, take one of our student’s particular cases. He had to go to seven dermatologists before anyone could diagnose him correctly, and that’s because he brought it up and practically self diagnosed himself! It might seem unbelievable that, even in this day and age, when medical breakthroughs are happening every day, that dermatologists are still quite uneducated about something as simple as hair health conditions. In essence though, Telogen Effluvium happens when there is a change in the number of hair follicles growing hair. In a normal scalp that isn't affected by Telogen Effluvium, there are approximately 100,000 hair follicles in your scalp and 10 % of those follicles are in the TELOGEN Phase, while 1% of those hairs are in the last days of its phase. Therefore, it is perfectly normal to shed between 50 to 100 hairs a day. You may not even notice these hairs as they come out throughout the day. However, in cases that involve Telogen Effluvium, a significant percentage of your hair follicles in the ANAGEN phase suddenly switch to the TELOGEN phase. Genetics The different factors you may have read about can make it confusing, but female pattern is the most common type of hair loss, without question. While certain lifestyle factors can have an impact on your hair’s thickness, before you start blaming your diet or blow dryer, get to know the facts behind the science of genetic hair loss. Female pattern hair loss, also known as androgenetic alopecia (AGA) or hereditary hair loss, is an inherited condition, which reduces the time that the hair spends actively growing. AGA causes the hair follicles to become smaller and slowly, thinner, lighter hair replaces your usual hair. Female pattern hair loss is a general loss of the hair, with the most prominent hair loss happening on the top of the head and along the part. What to do about female pattern hair loss? 2. Stress Stress or a traumatic event can cause temporary hair loss, or telogen effluvium. The effects of this can push significant numbers of your follicles into a resting phase. In the case of shock, sudden shock hair loss may be noticeable one to two months after a shock. Stress-related hair loss does not have to be permanent. Alopecia Areata Another non-hereditary hair loss cause is alopecia areata; this autoimmune disorder results in totally smooth, round hairless patches on the scalp. Over styling Braids, ponytails, cornrows, and extensions can all pull on your hair, resulting in hair loss that can lead to a condition known as traction alopecia. This video gives insight into the hundreds of hair disorders medical dermatologist treat. Disclaimer- I am a procedural dermatologist – this means I only perform procedure such as PRP and low level laser. I work with a team of medical dermatologist to treat hair loss disorders. As a rule I do not prescribe drugs as this needs careful monitoring at regular intervals. This video will concentrate on the management of female hair loss, a very different entity and approach to male pattern hair loss. In males, the most common condition by far is non scarring androgentetic alopecia, or genetic balding. Unfortunately I suffer from this condition – due to my father’s genes. In females androgenetic alopecia is much less common, and other medical conditions such as thyroid disorders, iron, Folate, vitamin deficiencies need to be investigated. Inflammatory disorders such as lupus, lichen planus or frontal fibrosing alopecia, and alopecia aerata need to be excluded. There are over 210 causes of hair loss, and this short video will only cover telogen effluvium and androgentic alopecia. Treatment options for female pattern hair loss due to androgentic causes include the following. 1. Minoxidil topically or orally. If a foam or lotion is to be considered, both male and females should use 5% at night. If oral medication is indicated by your medical dermatologist, 1-3 mg daily should be taken. You can obtain a bespoke prescription from your physician at www.myskinpharmacy.com.au For females this should be stopped in pregnancy as it not category A 2. DHT inhibition is another option- either Finesteride or Duasteride. Side effects and the risk benefit ratio will be discussed with your medical dermatologist. This is off label use in females, and must be ceased if you are pregnant. 3. Spironolactone or C. acetate are androgen blockers, and can be very useful in the management of female pattern hair loss. Your dermatologist will guide you as to the dose. 4. PRP or Platelet Rich Plasma can be adjunctive therapy. My protocol is to inject in the areas of Norwood or Ludwig pattern baldness. 3 sessions spaced 6-8 weeks apart, followed by 2 injections annually. As stated this is NOT a stand-alone procedure. This should be conducted by a specialist and not a nurse led procedure as the pattern of hair loss is critical to results. 5. Low Level Laser emission devices in the 630-655 nm wavelength can stimulate blood flow and increase hair density. Like PRP for hair loss this is not a stand alone procedure, and needs to be combined with medical or surgical procedures. My Laser of choice is the Lutronic System as it provides exact energy delivery to the entire scalp. This is a medical and not a cosmetic laser. 6. Surgical options. FUE is the current standard of practice. Your result will be dependent on one person- the surgeon. This is not a branch, brand or franchise, nor is it a robot. Do your research and find the best specialist. FUE is different for females than it is in males due to the pattern of hair loss. I do one video a week, covering common medical conditions that general dermatologist see, however my specialist trade is a laser, and procedural based dermatologist, with an interest in facial rejuvenation and acne scarring.
