http://www.acceleratemypractice.com/ Best Practices in Dental Office Scheduling When looking at the best practices for how to arrange your schedule in a dental office, I want to start with an exercise that Stephen Covey did years ago. He filled a jar with rocks and then asked if the jar was full - some naturally said "yes". Then he added small pebbles to the jar and they fell down in the cracks and filled some additional space. He then asked if the jar was full - some said "yes". He then added sand, which fell down in the cracks and filled the empty spaces and then asked yet again if it is full. Some again said "yes". He then added water to the jar, which of course filled the extra spaces, to which he now agreed "now it's full". If, on the other hand, you were to fill the jar with sand first and then ask the audience if it was full of course all would say "yes". With this scenario in mind, he then posed the question of, "now could you add any rocks to this jar?" The obvious answer was "no". You have no left over space to put anything into the jar. Now if you imagine the rocks to be the larger more profitable procedures and the sand to be emergencies and other smaller procedures, it becomes obvious why someone would fill the jar with the big stuff first. The same is true of your dental office schedule. It would be wise to first put the rocks in your schedule, then the pebbles, then the sand, and then the water to best fill your schedule. This takes dedication and leadership to accomplish. Here is how I would suggest you make this happen in your office if you don't currenlty do this. Start by envisioning your perfect day. What procedures are you doing? When would you like to do them and how many? Write this out. Now if this makes you nervous to make this change, start small. Block out a half day a few weeks out and stick to the plan. What will most likely happen is the day will fill up just like you envisioned, and you will finish the day, or half day, and then sit back thinking "WOW, that was fun and profitable." Now you will have the confidence to book another day like it. We find when you Start With the End in Mind, and then execute on a plan, often you get the desired results. I am not promising you 100% results, but my experience is that more often than not, you will get the results you desire. Contrast this with no plan and the results from that?? Every member of the dental team plays a role in keeping the office motivating but as the doctor, there are more eyes on you. Have you let your team slip through your fingertips and you constantly find yourself looking for ways to keep them motivated and loving their job?! You're thinking too much about it! Lets break this down for you. Team incentives are very important in a productive and successful work environment. To start, let’s take a look at what works and what does not. The first type of incentive that does not work is the one that has a long duration before the team is to receive a reward. The 6 – 12 month plan is an example of what may cause most teams to show a lack of interest in the end goal. It is imperative that the doctor put an immediate end to any incentive plan that turns to one of “entitlement.” If any staff member begins to say anything such as, “We deserve” or “We should have…” this is immediate grounds for dismissal of the incentive plan! It is a clear sign that it is no longer working. Examples of incentive plans that do work are ones that have value to the team. Ask your team what they would like as an incentive. By asking, this does not mean that you will necessarily choose what they request. It will give you an inside view of their thoughts and wishes. Ask what they are willing to do “extra” to achieve a fair and equitable bonus. Some short term bonus ideas can include setting a goal for: the number of whitening procedures done in 1 month the number of crown preparations done in 1 month the number of Cerec crowns completed in 1 month the number of new patients seen in 1 month the dollar amount collected prior to the commencement of treatment The reduction of “X” amount of accounts receivable over 90 days Bonuses can be gift cards, cash, a paid day off during the holiday time period, or something as simple as a nice lunch delivered to the office from a favorite team restaurant. One plan that worked superbly was an end of month 3 day bonus that included producing an additional $10,000. The doctor was absolutely shocked once he announced the “3 days from now bonus” at how the team pulled together in such a short time to each earn a $100. gift card of their choice. Never estimate what your team can do! Keep it short and sweet!

The last thing we want is to have you build your dream practice and then have it crumbling down because of government intervention and penalties. So I’ve said a lot about this, but you know what I can’t talk nearly as well about the technical as you, Jill. How are you? Jill: I’m doing awesome, Justin. It’s really nice to be here. I’m so excited to talk to your audience and it was great when I heard from you when you reached out, you know, my favorite topic is compliance and HIPAA is really the new OSHA. So there’s so many things that doctors need to be aware of. Not only the doctor, but also their team as well with regards to OSHA and HIPAA. Justin: Excellent and you know I’ve done a little vetting of, you know. I’ve seen you on dental town. I’ve heard good things about you and I’d love to just dive into this. You know, where do we start? Let’s talk about, you know, what is HIPAA? What kind of history and evolution and meaning of HIPAA? Jill: HIPAA, so HIPAA started in the 90’s and it really stands for Health Insurance Portability and Accountability Act of 1996. So right around that time when people were moving, you got transferred or you were moving. Insurance companies were making insurance policies skyrocketing price. So originally the president puts a cap on any insurance company being able to price gouge. Later it sort of morphed itself by the year 2000 to have a right to privacy. Many executives were going to interview for jobs and they were getting discriminated against if the big bosses were checking into their health history. Do they have any communicable disease and they were discriminated so, then the government intervened that, somewhere around 2003, all healthcare facilities including dental offices have to have patients sign a HIPAA Patient Acknowledgement form and really what this was, was a pledge from the doctor not to give away, sell their mark, their health information for marketing purposes like name, address, phone number. We were just getting into email, that sort of thing and today HIPPA protects any individuals- past, present, or future protected health information- PHI. There is a ton of acronyms in the HIPPA world. So PHI is what a doctor and his team really need to be aware of, they need to know what is HIPAA? What does that mean for the dental office? That’s a little bit of the history and the evolution of HIPAA but now it has changed and it accelerates so quickly. These days, you know when we think about 2003 we started with HIPAA signing simple forms, we’re going to keep everything private.

Governor Chris Christie responds to a teacher's question during a town hall meeting at Raritan Township. Discusses the teachers' union and the need for shared sacrifice. September 8, 2010. (transcript below) Q: Hi my name is Marie Corfield, I'm a teacher across the street and I wonder how your reforms are going to help the middle class when so many middle-class teachers have been laid off this year and so many middle-class teachers are spending tons of money out of their pockets to supplement the budgets that were cut so they can buy supplies and so many parents are having to outlay more and more money now for programs that were originally paid for by the school districts. And I'd also like to say New Jersey has some of the best schools in the country, and this administration has done nothing but lambaste us and tell us what horrible schools we have. We have some failing schools. But the majority of our schools are wonderful and fantastic and this school district has many of them. And I feel like you have alienated so many people with your rhetoric about how we are bilking this state of all this money when I'm a taxpayer and because of the budget cuts that you have implemented, I am going to have a hard time paying my bills this year. Governor Christie: Well, a few things. First of all, I have not lambasted the public school system in the state of New Jersey. What I have lambasted—well listen, let's start with this: I sat here, stood here, and very respectfully listened to you. If what you want to do is put on a show and giggle every time I talk, well then I have no interest in answering your question. So if you'd like to conduct a respectful conversation I'm happy to do it. If you don't please go and sit down and I'll answer the next question. What's your choice? All right. First, the cuts that we made this year were the result of a loss of a billion dollars in federal aid that was used as a one-time hit to artificially boost education funding in New Jersey. I didn't have that billion dollars to spend, President Obama sent it one time and Jon Corzine spent it all in one year, in his reelection year. So, I had a $1 billion hole to fill in the state education budget with declining state revenues. Despite that, I only cut 820 million from the budget which means that we spent an additional $280 million in state tax funds on K-to-12 education, more than was spent by Jon Corzine in the year before. So this idea that somehow I cut the state support of K-to-12 education is wrong. We lost a billion dollars in federal funding. Second, my lambasting and my rhetoric is directed very clearly at one set of people, and that is the leaders of the teacher's union in the state of New Jersey. They were given an option in a time when everyone in this economy is suffering, when we have had people in the private sector who have been out of work for years, and are continued to expect to be paying their property taxes or they will lose their homes, I asked teachers across the state of New Jersey to take a one-year pay freeze and to contribute 1 ½% of their salary towards their health benefits for those who didn't contribute already. The teacher's union response was that this was the greatest assault on public education in the history of the state. Ask the people in the private sector in the state of New Jersey when the last time was they got a raise. Yet the average teacher contracts before I became Governor had 4.9% annual increases in an era when we are at zero or 1% inflation. Now that can't be justified any longer. Next, the health benefit costs in the state of New Jersey for everyone in this room are out of control, and if you look at it the federal government for federal employees pays 66% of the cost of the health benefits for their employees. The state of New York pays 83% of the health costs for their employees. In the state of New Jersey, overall for all employees across the board, we pay 92% of the cost of the health insurance for every one of our public employees, and among teachers, the overwhelming majority of teachers in the state of New Jersey pay nothing towards their health insurance premiums for full family medical, dental, and vision coverage and for benefits that do not just continue during employment but if they stay employed long enough, for benefits that continue for life. Now listen, maybe those promises were appropriate to make decades ago. They're unaffordable now, and so my ask was a 1 ½% contribution, and I was told that I was destroying public education in New Jersey to ask teachers to make that contribution, 1 ½% of salary, which for the average teacher who makes $55,000 in New Jersey, you're talking about $750 a year for full family medical coverage...
